Procedures to Register a New/Returning Student to Wake County Schools

Parents/Guardians will need to bring the following items to their enrollment sessions in order to expedite the enrollment process of their student.
1. Proof of Residence: Current gas, electric, or water bill (PO Boxes are not accepted) or a Mortgage/Lease signed within the last 45 days. 2. Original or certified copy of the student's Birth Certificate. 3. An unofficial transcript from the previous school. 4. A withdraw form from the previous school (if in mid-semester of the school year) 5. Immunization Records (if not on transcript) 6. State law requires every child entering a public school in N.C. for the first time to receive a health assessment. This assessment must occur within 12 months prior to entering school. The medical parent or guardian must provide a completed health assessment form to the school on or before the child's first day of attendance.** 7. To be present for the enrollment session along with the student. (A stepparent, grandparent, family friend, etc cannot enroll a student in school) 8. Any custody documents 9. If student currently has an IEP/ 504 plan/ Health Plan- Bring the latest copy.
Students enrolling from a Home Schoolsetting must meet the regular enrollment requirements for all students (see above), and, Home School parents must also provide the following documentation on behalf of their student(s): 1. Enrollment Guide (click here to download the form) 2. Dates of Attendance- one sheet for each academic year (click here to download the form) 3. Materials Used- a form listing names of courses with their books/materials used, total number of instructional hours, and dates of enrollment; one sheet per academic year (click here to download the form) 4. Curriculum- the title of each course completed followed by a written course description 5. Home School Transcript 6. Home School Name, Address and Phone/Fax/Email 7. Parent Name(s), Address, Phone/Email 8. Student Name, DOB 9. Academic year with grade level, course names, semester/quarter grades, final grade, credit issued for each course, grading profile (ex. A=90-100, etc) and parent signature (signifying an official transcript) 10. Standardized test scores from each academic year from a nationally normed test [Iowa Test of Basic Skill (ITBS), California Achievement Test (CAT), Stanford Achievement Test (Stanford), etc)]
Wake County Policy also states the following with regard to incoming students: Grades will be awarded for courses completed at a school that is accredited by the Southern Association of Schools and Colleges or any of the other five regional accrediting associations. Grades of Pass/Fail may be awarded for courses completed at a school that is not accredited by one of the six regional accrediting associations. Grades of Pass/Fail are not included in the calculation of GPA or rank but are included on the student transcript as credit towards promotion and/or graduation.
**Parents whose children do not have a regular physician can get health assessments and immunizations through the Wake County Health Department. First-time appointments should call (919) 250-3947.
